Utility Service Disconnect

If you currently have service through Ceredo Water Works and would like to discontinue service, please use the form below to do so. A valid phone number and email address are required for confirmation.

AMERICAN WATER CUSTOMERS:

If you currently have service through American Water, you must contact them to disconnect your water service. We DO NOT forward your request.

DEPOSIT REFUND (IF ANY):

All account deposits, if any will be issued as a refund check mailed AFTER your account has been paid IN FULL. We do NOT apply this deposit to your account. We will mail the check to your forwarding address supplied above.

FINAL BILL:

Once your account has been disconnected and a final reading has been read, you will receive a final bill. The amount due must be paid in full or you risk the account being turned over for collections by a third party collections company. If this happens, you will incur additional fees from this company.

AGREEMENT:

By continuing further and completing this form, I agree under penalty of perjury the information provided above is accurate to the best of my knowledge. I also understand that I am responsible for all charges accumulated on the below referenced account until it has been disconnected, regardless of the date I requested. I also understand that requests are not processed after regular business hours (3:00 PM), holidays, or weekends. A representative will contact me at the phone number provided to confirm the disconnect and if the number is not valid or I am unable to be reached, this request becomes null and void. A final bill will be sent once a final reading has been completed and any deposit due will be forwarded to the address provided below by check AFTER my account balance has been paid in full. I also understand Ceredo Water Works may, at its discretion, turn over any unpaid balance to an outside collection agency.
